The nerves that give you sensation and the ducts that carry milk sit in the same small area, just beneath the nipple. That one anatomical fact is behind both of the questions women bring me before a breast augmentation. Will I still feel anything, and will I still be able to feed a baby? How are milk ducts and nipple sensation protected during breast augmentation?
Both of those structures sit close to where we work. The nerves that carry sensation to the nipple and the ducts that carry milk run through the same region, just beneath it. The less they are disturbed during an operation, the higher the chance that sensation and milk supply stay as they were.
What happens to the nerves in a classic breast augmentation?
In classic breast augmentation, tissue above or below the muscle is cut to create a pocket, and the implant goes into that pocket. When the operation is done in the submuscular plane, the muscle fibers are divided. That step can bring the work closer to nerve tissue. In some patients this shows up afterward as a temporary or permanent reduction in nipple sensation.
What does the Preserve technique do differently for nipple sensation?
With the Preserve technique the implant is placed above the muscle, in a space created without cutting through the tissue. Because the muscle stays whole and the handling of tissue is more limited, there is less chance of nerve damage. That is the mechanism behind the better odds of keeping sensation in and around the nipple.
The same logic applies to the milk ducts. What I want to be exact about is this: breastfeeding isn’t affected by either method. In classic breast augmentation and in the Preserve technique alike, the milk glands and the ducts are left untouched. The operations where breastfeeding really can become a question are breast reduction and breast lift surgery, because there the incision runs around the nipple.
Can the Preserve technique solve every problem in the breast?
No surgical technique can promise a hundred percent. The Preserve technique lowers the chance of bleeding, protects the nerves, and because it doesn’t cut the connective tissue of the breast it offers a more stable result in the long run. Other things shape the outcome too: your own anatomy, the current structure of your breast tissue, the size of the implant and the angle it sits at.
The accurate way to think about it is that the Preserve technique reduces some of the risks that come with breast augmentation.

We assess the thickness of your breast tissue, your current level of sensation, and whether breastfeeding is in your plans. From all of that we work out how much the Preserve technique would actually do for you. No two women have the same anatomy, so the only way to know whether the technique suits you is an examination.
How does nipple sensation change after breast augmentation?
A temporary numbness in and around the nipple is common right after surgery. It comes from tissue swelling and from the nerves reacting to the operation. It doesn’t mean permanent nerve damage. With the Preserve technique that numbness usually starts to fade sooner than after a classic augmentation, because there has been less handling of nerve tissue.
Sensation coming back completely can take weeks, sometimes months. The pace is individual, and it is something we follow at your regular check-ups. Try not to judge the final result by what you feel in the first few weeks.
