Which one is right changes from patient to patient, and it changes for reasons you can actually see on an examination table.

What is the basic difference between Preserve and classic breast augmentation?

In classic breast augmentation the implant goes under the muscle, or into a pocket created by cutting the tissue above the muscle. In the Preserve technique the implant is placed above the muscle, and neither the muscle nor the breast tissue is cut. The Cooper ligaments and the circummammary ligament that give the breast its shape stay intact, and we expect that to produce a more stable result over the years.

Which patients does the Preserve technique suit?

In women whose breast tissue is thick enough, the Preserve technique is usually the option I prefer. Because there’s less trauma, it means a shorter recovery, less pain and a faster return to normal life, and for patients with those goals it has become a strong choice.

Women with an active lifestyle, women who train, and women who want to be back at work quickly are generally very happy with it.

Why is classic breast augmentation still chosen?

In patients whose breast tissue is thin, the classic method can be the safer option. The muscle acts as an extra layer of cover over the implant and softens how visible its edge is. Some women who want extra fullness in the upper part of the breast also do better with the classic approach.

And in patients who have had previous surgery, the state of the tissue can make the classic method the more suitable one.

Who decides between Preserve and classic, and how?

This decision shouldn’t be made from what you’ve read online. It’s made from what we find at the consultation. We assess the thickness of your breast tissue, the elasticity of your skin, what you’re hoping for and how you live. From all of that we work out which method will serve you better.

Both methods have their advantages and their limits. Declaring one better than the other misses the point. What matters is which of them fits this patient’s anatomy and priorities.

Is the Preserve technique automatically better because it’s newer?

“Newer means better” is a misleading assumption. The Preserve technique genuinely does help many patients, but that doesn’t make the classic method outdated or inadequate. The classic technique has been performed for decades and still gives excellent results in the right patient. The decision should follow your anatomy, not what happens to be popular.

How does recovery differ between Preserve and classic breast augmentation?

Leaving the muscle untouched is the difference you feel most concretely. In the classic method the first days can be harder because of the muscle incision, and getting back to normal life usually takes longer. With the Preserve technique that period is more comfortable for most patients, and the return to daily life is noticeably quicker.

Is there a long-term difference between Preserve and classic breast augmentation?
